As fertility dietitians, we see firsthand the anxiety and overwhelming feelings associated with this topic. For those who have endeavoured to lose weight prior to conception to often “become eligible” for IVF or egg freezing, the thought of getting to the start line and knowing weight gain is ahead can understandably be overwhelming!

So let’s delve into the scientific research, share our insights as experienced fertility dietitians, and address the common myths surrounding IVF medications and weight gain.

IVF: The Process of Controlled Ovarian Stimulation

IVF and egg freezing involve the administration of high doses of gonadotropin hormones through injections.

These medications stimulate the ovaries to produce more follicles, hopefully containing viable eggs for retrieval. It’s important to note that individual experiences with these medications can vary greatly, with some reporting mild or no side effects.

Weight Gain with IVF: How Much is Normal?

A study conducted in 2021 followed 358 women undergoing controlled ovarian stimulation protocols. The researchers examined changes in body weight before and after stimulation and also collected data on anxiety and binge eating.

On average, participants experienced an increase in weight of just under 400g (approximately 1-2lbs) from the start of stimulation to trigger day. This weight gain can be attributed to factors such as enlarged ovaries, follicular fluid accumulation, and water retention caused by the medications. (Tso 2021).

Interestingly, the study found that weight gain during stimulation was primarily related to increased food intake. But what about the anxiety and binge eating scores? Interestingly, anxiety and binge eating scores were not linked to weight gain in this particular study.

However, it’s important to consider potential reporting bias and the limited timeframe of the study. Additionally, the study concluded that weight gain during stimulation is often due to edema, which is temporary and expected to subside within a few days after the egg retrieval procedure.

Ovarian Hyperstimulation Syndrome (OHSS): A Serious Consideration

It is important to mention, Ovarian Hyperstimulation Syndrome (OHSS) is a potential complication where the ovaries respond a bit too well to the medications, causing swelling, pain, nausea, and significant fluid retention (Kumar, Sait, Sharma, & Kumar, 2011).

In severe cases, hospitalisation may be required. If you suspect OHSS after your IVF treatment, it’s crucial to consult with your doctor immediately.

Emotional Eating and Lifestyle Factors

During the IVF process, many factors can influence eating habits.

The combination of hormonal changes, anxiety, disrupted sleep, and fatigue can lead to emotional eating or seeking comfort in food. This emotional aspect, coupled with lifestyle changes, such as altered exercise routines or fear of exercise impacting the cycle outcome, can result in eating more than usual.

It’s essential to acknowledge these emotional triggers and also to find healthier coping mechanisms to avoid unnecessary weight gain, this is usually a good time to consider seeing an experienced psychologist with an interest in fertility and IVF.

To mitigate weight gain during IVF, it’s important to address emotional eating patterns well before starting the process. Seeking support from a fertility dietitian can be highly beneficial.
